Patients are seen by appointment only. Same-day appointments are available for individuals with urgent medical needs. Routine physical exams are scheduled in advance.

Please bring your insurance information with you and plan to arrive about 15 minutes early — 30 minutes for new patients — for your appointment, to complete necessary paperwork.

We make every effort to stay on time, and appreciate your patience should an emergency arise that puts us behind schedule.

If you are unable to keep your appointment, we ask that you cancel or reschedule at least 24 hours in advance, and 48 hours in advance for physicals. To make a new appointment or change an existing appointment, please call 602-371-3100.

Referral and Prior Authorization Information

Many managed care plans require a written referral or prior authorization for specialists, X-rays, lab procedures or other medical care outside our office. Our referral department can help you meet the requirements of your plan.

Please make sure you contact the referral department when in doubt. Please note that retro authorizations are often denied as all authorizations must be obtained before your specialist or procedure appointment.


Requests for prescription refills are handled only during regular office hours. Please call your pharmacy and ask them to fax a refill request to us at 602-371-0050. This saves time and provides accurate refills requests. Please allow at least 72 hours to complete your refill.
