Patients are seen by appointment. We offer convenient office hours Monday through Friday, and we will do our best to accommodate your schedule.

If you are unable to keep your scheduled appointment, please give us at least 24 hours notice.

Insurance and Payments

We accept most major insurance plans. If our practice is not listed among your plan's participating providers, please call our office to confirm that your health care coverage includes us.

We want to work with you to assure that the care you receive is in keeping with your insurance provider's requirements. Since you, the patient, are financially responsible for the cost of the care we provide, we encourage you to learn about your benefits and their limitations. It is also wise to be informed of any benefit restrictions due to pre-existing conditions.

Please bring your insurance card to every appointment and promptly report any changes in address, telephone or other personal information. Your insurance copay, unmet deductible and any other out-of-pocket costs are expected to be paid at the time of service. For your convenience, we accept major credit cards, in addition to cash and checks.

Referrals and Prior Authorizations

Many managed care plans require a written referral or prior authorization for specialists, X-rays, lab procedures or other medical care outside our office. Our referral department can help you meet the requirements of your plan.

If you need a referral or authorization, please call 602-485-7435 and allow 10 working days for your request to be completed.


Requests for prescription refills are handled during our regular office hours only. Please call your pharmacy and have them fax a refill request to us at 602-485-7416. Please allow at least 72 hours to complete your refill.
