Appointments

Patients are seen by appointment. Same day appointments are available for patients with urgent medical conditions. We make every effort to stay on time so your wait will be minimal. From time to time, emergencies may arise, which will put us behind schedule. We appreciate your patience during these times.

If you are unable to keep your scheduled appointment, please notify us as soon as possible.

To make a new appointment or change or cancel an existing appointment, call 602-997-7331.

After Hours/Emergency

A physician is available after hours by calling our answering service at 602-997-7331. If you have a life-threatening medical emergency, call 911 or go to the closest emergency room to get immediate assistance.

Insurance and Payments

We accept most insurance plans, including Medicare, Blue Cross/Blue Shield, UnitedHealthcare, and others. Please call your insurance company to confirm that your health care coverage includes us, and bring your insurance card to every appointment. Since you, as a patient, are financially responsible for the cost of the care we provide, we encourage you to learn about your benefits and their limitations. Please notify us immediately of any changes in your insurance coverage.

Patients with insurance are asked to pay their co-pays, co-insurance or deductibles at the time of service. If you do not bring your co-pay with you, you may be asked to reschedule your appointment.

If you do not have insurance, payment in full is expected on the day of service. A 20 percent discount will be given to self-pay patients if they pay at the time of service and do not have a prior balance. Special billing arrangements may be made with our billing department when necessary.

We accept Master Card, Visa, American Express and Discover, in addition to cash and checks.

Prescriptions

Requests for prescriptions are handled only during regular office hours. Please call your pharmacy when you need a prescription refill and have them fax a refill request to us at 602-870-4512. They will contact our office for authorization. Please allow up to 72 hours to complete your refill.

If you need a new prescription, please call our office to make an appointment with one of our doctors.

Test Results

You will be contacted by our physicians or medical assistants via telephone or letter informing you of your test results. Most test results are available within 48 hours, but some can take up to 10 working days. If you have not heard from our office within 10 working days, please contact our office at 602-997-7331 and speak with your doctor's medical assistant.
