Patients can be seen by scheduling at appointment at 623-312-2265. Same-day appointments are available.

Insurance and Payment Information

We accept most insurance plans, including Medicare, Humana, UnitedHealthcare, Blue Cross/Blue Shield, third-party insurance, workers' compensation, and many others. If we are not listed among your plan's participating physicians, please call our office to confirm that your health care coverage includes our services.

We also accept all major credit cards including Discover and American Express.

Patients on insurance plans are asked to pay their co-payments at the time of service. Special arrangements may be made with our billing department when circumstances make it difficult to meet your financial responsibility at the time of your visit.

In order to get a prescription refilled for most medical conditions, you must have had an appointment with one of our physicians during the last 6 months. Please contact your pharmacy and have them fax a refill request to us at 623-312-2266 and allow at least two working days to refill your prescriptions.

If you need a new prescription or need to renew an existing prescription, please call 623-312-2265 for an appointment with one of our doctors.

Referrals and Prior Authorizations

Many managed care plans require a written referral or prior authorization for specialists, X-rays, lab procedures or other medical care outside our office. In such cases, we can help you meet the requirements of your plan.
