Our receptionists have been instructed to ask why you need to see the doctor. Your response will enable them to schedule the appropriate length of time for your medical needs. For your convenience, you will receive an appointment reminder call two business days prior to your scheduled appointment.
If it is necessary to cancel an appointment, please notify our office as soon as possible. This courtesy will provide us with an opportunity to care for another patient in your place. Our staff will be happy to arrange another appointment for you.
We request that all new patients arrive 20 minutes prior to the scheduled appointment so that you have time to complete your registration with our staff. The receptionist will ask you to complete a registration form in order to establish your medical record and obtain your current insurance information. We ask that you bring your current insurance information and driver's license with you to this visit and all subsequent visits, as well as your co-payment, if applicable.
On future visits, please arrive 15 minutes prior to the check in time. Our receptionists will review your information for accuracy and request updates as needed. You can help us serve you better by notifying the receptionist of any changes in name, address, telephone number or insurance coverage since the time of your last visit. Verifying this information at each visit will ensure that you receive the most appropriate medical care, and also ensure that the services you received are covered by your insurance company.
Clinicians as John C. Lincoln Your Family Medicine try to follow their scheduled appointments as closely as possible. However, due to unavoidable circumstances or emergencies, a doctor may have to spend additional time with a patient who may have had an appointment prior to yours. This may result in a delay in seeing your doctor. We appreciate your patience and understanding in such circumstances.
To assure accuracy, please make sure your insurance, address, and contact information are up-to-date. Usually all the health plan information needed is found on your insurance card. Please familiarize yourself with the coverage provided by your particular insurance plan. In addition, bring your insurance card with you so that we can copy and file it in your Electronic Health Record. We participate in most insurance plans.
Medication requests are reviewed and expedited by appointment only. We reserve the right to fill or deny your prescription renewal request. If your request is denied, our practice will contact you directly to explain the reason for the denial.
Note: A limited refill of routine maintenance medications may be approved prior to an appointment due to extenuating circumstances.
Controlled or narcotic refills are handled by appointment only.
Referrals & Prior Authorizations
We have some of the best referral specialists in the business, to help coordinate your medical needs outside our office. Some insurance companies allow their clients to see their own choice of specialist without a referral. In that case, we would appreciate it if you notify the consulting specialist that we would like to have copies of their notes and tests, so that we can properly coordinate your care.
We always attempt to provide referrals to excellent consulting specialists who are on your plan. If they are no longer contracted with your health plan, call your health plan for an alternative referral. You may use our referrals with any provider you choose, who is on your plan.
