For Our Patients


Patients are seen by appointment. Same-day appointments are available for individuals with urgent medical needs. Walk-ins are welcome and will be seen as quickly as possible. Please bring your insurance information with you and plan to arrive about 20 minutes early for your appointment to complete necessary paperwork.

If you are unable to keep your scheduled appointment, we ask that you call to cancel or reschedule at least 24 hours in advance.

Insurance and Payments

We want to work with you to assure that the care you receive is in keeping with your insurance provider's requirements. Since you, as the patient, are financially responsible for the cost of the care we provide, we encourage you to learn about your benefits and their limitations, including any benefit restrictions due to pre-existing conditions.

Payment is required at the time of service. We ask that patients covered by insurance plans pay any required co-payments, co-insurance, deductibles and/or non-covered services at the time of their visit.

If circumstances make it difficult to meet your financial responsibility at the time of your visit, special arrangements can be made with our billing office. Please call 602-485-7421 with questions about payments and billing.


Requests for prescriptions are handled during our regular office hours of 7:00 a.m. - 12:00 p.m. and 1:00 p.m. - 5:00 p.m., Monday through Friday. Please call your pharmacy and have them fax a refill request to us at 480-342-7077. Please allow at least 72 hours to complete your refill.


Many managed care plans require a written referral or prior authorization for visits to specialists, X-rays, lab procedures or other medical care outside our office. Our referral department can help you with the requirements of your plan. Please allow 10 working days for the completion of the process.
