Patients are seen by appointment. Please arrive about 15 minutes early for your appointment (new patients should arrive 30 minutes early to their first appointment) in order to complete all necessary paperwork. Please bring your insurance information with you, as well as any relevant X-rays, diagnostic test results or referral forms.
Routine well physical exams are scheduled several weeks in advance. We make every effort to provide you with the appointment day and time that work best for your schedule.
Same-day appointments are available for individuals with urgent medical needs.
We make every effort to stay on time so your wait will be minimal. From time to time, emergencies may arise which will put us behind schedule. We appreciate your patience during these times.
If you are unable to keep your scheduled appointment, our office requires at least a 24-hour notification.

Insurance and Payments
We accept most insurance plans, including Medicare, Humana, Aetna, Blue Cross/Blue Shield, third-party insurance, workers' compensation, and many others. We will file your insurance claim for you.
Here are a few things you should bring with you to your appointment:
- Your insurance card. We will make a copy of it for your patient chart.
- Co-payment. Most insurance plans require co-payment at the time of service. Our office accepts cash, checks, MasterCard, VISA, American Express and Discover.
- Any changes in your address or telephone number.
If you are not insured or need to meet a deductible, our staff will be happy to work with you to make payment arrangements.

Referrals and Prior Authorizations
Many managed care plans require a written referral or prior authorization for specialists, X-rays, lab procedures or other medical care outside our office. Some medications also require prior authorization. In such cases, we can help you meet the requirements of your plan.
If you need a referral or authorization, please call and allow us adequate time to complete this process.
Requests for prescriptions are handled from 7:00 a.m. - 5:00 p.m., Monday through Friday.
Please call your pharmacy and have them fax a refill request to us at 602-375-5510. Please allow at least 72 hours to complete your refill.
