For Our Patients


Patients are seen by appointment. Please arrive about 10 minutes early for your appointment (new patients should arrive 30 minutes early to their first appointment) in order to complete all necessary paperwork. Please bring your insurance information and medication information, including pharmacy location and telephone number with you, as well as any relevant X-rays, diagnostic test results or medical records.

Routine physical exams are scheduled several weeks in advance. We make every effort to provide you with the appointment day and time that work best for your schedule. Same-day appointments are available for individuals with urgent medical needs.

We make every effort to stay on time so your wait will be minimal. From time to time, emergencies may arise which will put us behind schedule. We appreciate your patience during these times.

If you are unable to keep your scheduled appointment, our office requires at least a 24-hour notification.

Insurance and Payments

Here are a few things you should bring with you to your appointment:

  • Your insurance card. We will make a copy of it for your patient chart.
  • Co-payment. Most insurance plans require co-payment at the time of service. Our office accepts cash, checks, VISA, MasterCard, American Express and Discover.
  • Any changes in your address or telephone number.

We work with most major insurance plans. Please note that not all doctors are contracted with each insurance company.

Test Results

There are different timelines for receiving results depending on the tests ordered. You will be contacted by phone via our voice messaging system and given instruction on how to retrieve your results. If you do not hear from our office within two weeks from your test date, please notify your doctor's medical assistant.


Please call your pharmacy when you need a prescription refill and give them at least 72-hours' notice. In order to get a prescription refilled for most medical conditions, you must have had an appointment with one of our physicians during the last 6 months. Your pharmacy will contact our office for authorization.

Please note that we do not provide refill authorizations on weekends, holidays or after hours. Pain medications will not be authorized over the phone.
