Patients are seen by appointment only. Same-day appointments are available for individuals with urgent medical needs.
Please bring your insurance information with you and plan to arrive about 15 minutes early — 30 minutes for new patients — for your appointment, to complete necessary paperwork.
We make every effort to stay on time, and appreciate your patience should an emergency arise that puts us behind schedule.
If you are unable to keep your appointment, we ask that you call to cancel or reschedule at least 24 hours in advance. To make a new appointment or change an existing appointment, please call 602-942-8512.
Because our office is sometimes cold, please bring a sweater.
Insurance and Payments
We accept most insurance plans, including Medicare, Blue Cross Blue Shield, UnitedHealthcare and others. Please call your insurance company to confirm that your health care coverage includes our physicians.
Bring your insurance card to every appointment. Since you, as a patient, are financially responsible for the cost of the care we provide, we encourage you to learn about your insurance benefits and their limitations. It is also wise to be informed of any benefits restrictions due to pre-existing conditions and if physicals/wellness exams are covered.
Patients with insurance are asked to pay their co-pays, co-insurance and deductibles at the time of service. If you do not bring your co-pay with you, you may be asked to reschedule your appointment.
Please notify us immediately of any changes to your insurance coverage.
For your convenience, we accept Master Card, Visa, American Express and Discover, in addition to checks and cash.

Referral and Prior Authorization Information
Many managed care plans require a written referral or prior authorization for specialists, X-rays, lab procedures or other medical care outside our office. Our referral department can help you meet the requirements of your plan.
If you need a referral or authorization, please call 602-942-8512. Allow 10 working days for your request to be completed.
Requests for prescription refills are handled only during regular office hours. Please call your pharmacy and ask them to fax a refill request to us at 602-942-1075. Please allow at least 72 hours to complete your refill.
