Our patients are seen by appointment and we also do our best to accommodate same-day emergency visits. We make every effort to see you in a timely manner.
If your health plan requires an authorization or referral, please obtain this from your primary care physician prior to your appointment.
If you are a new patient, please arrive 30 minutes early to complete necessary paperwork. To make, change or cancel an appointment, call 602-264-7630.
Insurance and Payments
We accept a wide array of insurance plans. Please contact our office for a list of the insurances we accept.
We do ask that you bring your insurance card to each appointment.
Since you, as a patient, are financially responsible for the cost of the care we provide, we encourage you to be familiar with your insurance benefits and any coverage limitations you may have. It is also wise to be informed of any benefit restrictions due to pre-existing conditions and whether physicials or wellness exams are covered.
Patients with insurance are asked to pay their co-pays, co-insurance and deductibles at the time of service. You may be asked to reschedule your appointment if you do not bring your co-pay with you.
For your convenience, we accept MasterCard, Visa, American Express and Discover, in addition to cash and checks. Please call us at 602-264-7630 with any questions related to insurance or billing.
Referrals and Prior Authorizations
All AHCCCS plans as well as most managed care plans require a written referral or prior authorization for a patient to see a specialist. If your insurance plan requires a referral to a specialist, we will need your referral on file prior to your appointment. You will need to contact your primary care physician to obtion the referral, which can be faxed to our office at 602-264-5803.
Please call your pharmacy first when you need a prescription refill. The pharmacy will then fax the refill request to our office. Please allow our office 72 hours to process your request.
In an effort to have a prescription refilled for most medical conditions, you must have had an appointment with one of our physicians during the last 6 months.
