For Our Patients


Please arrive 20 minutes prior to your appointment and bring with you the following:

  • An updated list of all medications prescribed to you.
  • A list of over-the-counter medications you are taking.
  • Photo identification and insurance card.
  • Any changes in your address or telephone number.

Same-day appointments are available for individuals with urgent medical needs.

Insurance and Payments

We accept most major insurance plans, including Medicare, Cigna, Blue Cross Blue Shield, UnitedHealthcare, AHCCCS Mercy Care, Care 1st and APIPA. We also accept workman's comp, discount and fee-for-service plans. Please call our office at 602 938-8150 for additional information or inquiries.

Most insurance plans require co-payment at the time of service.

Our office accepts cash, checks, MasterCard, Visa, American Express and Discover. If you are not insured or need to meet a deductible, our staff will be happy to work with you to make payment arrangements.


Requests for prescriptions are handled from 8:00 a.m. to 4:30 p.m., Monday through Friday. Please call your pharmacy and have them fax a refill request to us at 602-938-9277. Please allow at least 72 hours to complete your refill.

Referral and Prior Authorization Information

Many managed care plans require a written referral or prior authorization for specialists, X-rays, lab procedures or other medical care outside our office. Some medications also require prior authorization. In such cases, we can help you meet the requirements of your plan.

All patients requiring pain management are referred to pain management specialists.
