Patients are seen by appointment only. Same-day appointments are available for individuals with urgent medical needs. We make every effort to stay on time, and appreciate your patience should an emergency arise that puts us behind schedule.
Insurance and Payments
We accept most insurance plans, including Medicare, Humana, Blue Cross/Blue Shield, third-party insurance, workers' compensation, and many others. If we are not listed among your plan's participating physicians, please call our office to confirm that your health care coverage includes our services. We also accept MasterCard and Visa.
Payment is required at the time of service. Patients on insurance plans are asked to pay their co-payments, co-insurance or deductibles as well as any non-covered services at the time of their visit. Special arrangements may be made with our billing department when circumstances make it difficult to meet your financial responsibility at the time of your visit.

In order to get a prescription refilled for most medical conditions, you must have had an appointment with one of our physicians during the last 6 months. Please contact your pharmacy and have them fax a refill request to us at 602-381-1341 and allow at least 2 working days to refill your prescriptions.
If you need a new prescription or need to renew an existing prescription, please call 602-955-6632 for an appointment with one of our doctors.
Referrals and Prior Authorizations
Many managed care plans require a written referral or prior authorization for specialists, X-rays, lab procedures or other medical care outside our office. In such cases, we can help you meet the requirements of your plan.
