Community Health Needs Assessments

Community service is the cornerstone of the John C. Lincoln Health Network, the owner and operator of Deer Valley Hospital and North Mountain Hospital.

The network began in 1927 as Desert Mission, with the goal of addressing the health and social needs of struggling families in North Phoenix. Today, we are a vertically integrated health care system dedicated to building healthy communities. One thing remains constant: We are guided to provide high-quality health care services focused on community benefit.

As the following Community Health Needs Assessment reports demonstrate, John C. Lincoln Health Network is known for anticipating community needs and adapting to meet them.

Affiliation

In October 2013, John C. Lincoln Health Network reached an agreement to affiliate with the locally-owned, non-profit hospital organization, Scottsdale Healthcare. As a result, changes have occurred that impact the published Community Health Needs Assessment Implementation Plans for both legacy organizations. The health priorities identified have not changed. Please see the original reports (above) for the methodology used to develop the plan and the modified reports for any changes to the original reports due to the affiliation.
