To review the list, click on the appropriate age grouping of your child. Your child will be identified by the last two digits of the custodial parent's or legal guardian's home phone number followed by the first initial of the first and last name of the mother, or the legal guardian. When there is no phone number or when the last two digits of the number are 00, the digits 00 will be used followed by the mother's or legal guardian's initials, as stated above.
