Insurance

John C. Lincoln Health Network knows that when it comes to you and your family, nothing is more important than your health. That's why we provide employees with outstanding insurance benefits and options. Our employees become eligible for the benefits listed here on the first day of the month following their date of employment, unless otherwise noted. (For example, an employee who starts work on April 6 becomes eligible on May 1.)

Eligibility

John C. Lincoln Health Network provides benefits to employees who are hired to work 30 hours or more per week. Eligible dependents include: legal spouse, natural children, step children who reside with the employee more than 8 months during the year or whose spouse has court-ordered responsibility to maintain health insurance, domestic partner, and children of domestic partner (except dependent life insurance). Children will be covered up through the month of their 19th birthday. They can continue coverage up through the month of their 25th birthday with the medical, dental, vision, and dependent life insurance plans if they are a full-time student. Documentation is required to enroll domestic partners.

The following benefit examples are for non-exempt employees hired to work 30 or more hours per week. Some benefit levels may vary for part-time employees.

Medical Insurance

Blue Cross Blue Shield (BCBSAZ)

Blue Cross Blue Shield (BCBS) of Arizona is a Preferred Provider Organization (PPO). The network subsidizes a portion of the premiums for employees and dependents. Each time they need medical care, employees and their families insured through BCBS have the choice of using providers in the PPO Network or going Out-of-Network. Employees may choose to use the PPO Network provider as the most cost-effective option or Out-of-Network provider for treatment with additional out-of-pocket costs. In addition to medical coverage, urgent care services, hearing benefits, chiropractic benefits and a prescription plan are included.
 
Employees may also choose the High Deductible Health Plan (HDHP) with a Health Savings Account (HSA) option.  The HDHP has a higher annual deductible than typical health plans.  It also has a maximum limit on the sum of the annual deductible and out-of-pocket medical expenses that participants will pay for covered expenses.  HDHP insurance does not cover first dollar medical expenses, except for preventative care. Participants will choose from the BCBS PPO network or an out-of-network provider.
 
An HSA is a special account owned by the participant where contributions to the account are used to pay for current and future medical expenses.  An HSA allows you to deposit money on a pre-tax basis up to the IRS limits.  This can be savings from lower premiums, additional participant funds, and matching funds from JCL.
 
Employees may also opt to waive their medical insurance. Employees who choose to do so can receive "flex" credits, which can be used to purchase additional benefits or be added back to their pay.

Dental Insurance

Employees may choose from three dental options: Assurant Dental, which is a dental HMO, and two Delta dental plans. Delta is a traditional indemnity dental option. John C. Lincoln Health Network subsidizes a portion of all three plans. Employees who choose to waive their dental insurance can receive "flex" credits. The "flex" credits can be used to purchase additional benefits or be added back to their pay.

Vision Plan

John C. Lincoln offers two vision options: a standard vision plan, and a deluxe vision program.  These vision benefits are provided through Spectera.  Employees may use a wide network of providers.  Both vision options offer discount Lasik surgery options.
 

Life Insurance

A term life insurance policy of $25,000 is provided at no cost to benefit-eligible employees. Employees can also purchase supplemental life insurance benefits up to five times their salary or $600,000, if they choose. The supplemental premiums are based upon age and annual base salary.

Dependent Life Insurance

A nominal premium covers all eligible family members for dependent life insurance, from $5,000 to $50,000 coverage for spouse and/or $2,000 to $10,000 coverage for eligible children.

Income Security Plan (ISP)

The ISP plan is available to all employees who are benefit-eligible and have been with Lincoln in a benefit-eligible position for at least six months. Coverage will begin after you have missed 32 hours of scheduled work shifts.

The Income Security Plan will pay 66 2/3 percent of your salary for up to 180 days, at which point if you are still disabled, you would apply for long term disability coverage. This coverage will increase the longer you are with Lincoln.

Long Term Disability (LTD)

Long term disability provides income protection in the event of a disability. John C. Lincoln Health Network provides, at no charge, a benefit level of 50 percent of an employee's pay should he or she become disabled. Increased protection from 50 percent to 60 percent of an employee's salary is available at an additional cost to the employee.

Flexible Spending Accounts

Dependent Care

Benefit-eligible employees can choose to have a portion of their pay deposited into a dependent care flexible spending account. The account allows an employee to set aside pre-tax money for dependent care expenses, such as child care or adult day health care. Employees can set aside up to $5,000 on a pre-tax basis each year.

Unreimbursable Medical

Benefit-eligible employees can set aside up to $2,500 per year for expenses not covered by their health insurance. Examples of such expenses include co-pays, deductibles, braces, eyeglasses, contact lenses and pharmacy co-pays. This benefit is also available on a pre-tax basis.

In Lieu of Benefits (ILB)

This program allows employees to receive a higher rate of pay in lieu of benefits. Under this option, employees waive all pre-tax benefits, the 401(k) retirement plan, and/or earned time and ISP. Employees who elect one of the three options can receive between 7 and 20 percent additional pay. Benefits including the 403(b), 529 College Savings Plan, long term care, after-tax supplemental insurance, group legal, pet insurance, education assistance, discounted cellular phone service, gift shop/café discounts and credit union membership continue to be available to employees who chose an ILB option.
